Kidney Stones

What are kidney stones: Your urine contains many dissolved minerals and salts. When these minerals and salts increase in higher than normal level, the kidneys are unable to process them adequately, and thus these form as Kidney stones. As such, kidney stones are deposits of mineral or acid salts that form inside the kidneys or along the urinary tract. These deposits or ‘stones’ are excruciatingly painful, if not treated in time.

Who is likely to get kidney stones: While kidney stones are a universal phenomenon, they are more likely to occur within people of heavier weight. Obesity and diabetes are also strongly associated with a history of kidney stones Kidney stones impact nearly 10% of the population, and are especially common amongst people with predisposition to obesity and diabetes. Recent research suggests that lifestyle and dietary behavior play and important role in creating risk for the disease, with high association between stone disease and weight gain, BMI, and diabetes mellitus. The percentage of population suffering from kidney stones, increases dramatically between the ages of 40 and 65.

Calcium stones: Most common type of kidney stones, they can be of two types, viz., calcium oxalate and calcium phosphate. The risk of these rises, with the incidence of high calcium in urine

Uric acid stones: These can come if one is overweight, suffers from chronic diarrhea, has type-2 diabetes or high blood sugar, suffers from gout or people who consume a diet that is high in animal protein and low in fruits & vegetables.

Infection or Struvite stones: While these are not generally common, they tend to affect people with condition of chronic urinary tract infection.

Cysteine stones: These are a rare form of stones that are formed when kidneys do not reabsorb cysteine from the urine, which happens largely due to inherited metabolic disorders

Obesity: Being overweight or with skewed BMI, leads to changes in acid levels in urine, thus increasing the possibility of stones formation.

Diet: Excessive consumption of salt, and eating foods rich in oxalate increases the risk of forming calcium stones. High acid levels that are a result of high intake of food rich in animal protein (beef, poultry, meat, fish), which in turn make it easier for calcium oxalate and uric acid stones to form.

Low urine volume: Poor intake of water, loss of body fluids (such as during intense exercising) ensure that there is insufficient amount of fluids in the body to enable sufficient urine formation, which in turn means lower amount of salts dissolution.

Medical conditions: such as abnormal growth of the parathyroid glands that control the calcium metabolism, can cause disruption in calcium absorption, leading to high calcium levels in blood and urine, thus leading to kidney stones. Conditions that lead to acid build up, such as distal renal tubular acidosis, raise the risk of formation of calcium phosphate kidney stones. Certain inherited conditions such as Cystinuria leads to higher levels of amino acid cysteine in the urine, while another condition known as Hyperoxaluria, where liver makes too much oxalate.

Medications: certain medications, such as calcium supplements and Vitamin C consumption may increase the risk of stones formation. Be sure to tell your medical practitioner all medication and supplements you take, and ensure that all these are taken under medical supervision.

Family history: of kidney stones condition increases the chances of kidney stones amongst immediate family.

Chronic diarrhea: can lead to loss of body fluids, thus lowering urine volume. Additionally, there is the rick of absorption of excessive oxalate from the intestine, resulting in more oxalate in the urine. The combination of low urine volume and high levels of oxalate, may lead to formation of calcium oxalate kidney stones

Kidney stones start small, but can grow larger in size, and tend to even fill the hollow structures of the kidney, hampering the long-term function of the organ. This gives a sharp, cramping back and side pain, which progresses to the lower abdomen or groin area. This pain usually is sudden and comes back in waves as the body tries to get rid of the stones. Other symptoms may include

1 - Intense need to urinate

2 - Urinating often, and a burning sensation while urinating

3 - Urine that may be dark due to a mix of blood in it. While this is a common symptom, it may not be readily visible to the naked eye due to the small amounts of red blood cells in the urine

4 -Feeling nauseous or vomiting

It is important to seek medical advice to decide the course of action within modern medicine, which offers the following courses of treatment

Medication: Certain medicines, though not specifically for treatment of kidney stones, have been shown to improve the chance that a stone will pass while urinating. The most common is Tamsulosin that belongs to a group of medicines called alpha-blockers. This is generally used to treat men with symptoms of enlarged prostrate (known as benign prostate enlargement), and is also occasionally taken to relax the ureter, making it easier for the stone to pass. This, however, may also need pain and anti-nausea medicine while you wait for the stone to pass.

Surgery: is the prescribed solution in modern medicine if the stone fails to pass through the urinary tract. There are various types of surgery offered, and include ‘shock wave lithotripsy’, ‘ureterostoscopy’, and ‘percutaneous nephrolithotomy’. These are invasive solutions to removal of kidney stones, and may require administration of anesthesia and in some cases, overnight stay in hospital, as well as post-operation recovery time.

Ayurveda: Ayurvedic treatment of kidney stones rely on altering the biochemical quality of urine, which help in the dissolution of stones and further prevent stones formation. These diuretic herbs are indicated to help break the stones so they are easily expelled in urine. Additionally, in instances that patients have undergone surgery for removal of kidney stones, ayurvedic remedies help avoid recurrence of kidney stones.

Kidney stone mantra is useful in prophylaxis & treatment of adult and pediatric urolithiasis – kidney stone formation, including calcium oxalate stones, calcium and phosphate stones and, uric acid & urate bladder stones.
